AB OBJECTIVE: Diagnosis of underlying HIV infection may be difficult in
early neuromanifestations. In this study we tried to assess the role of
neopterin as a further tool for detecting HIV infection. METHODS:
Follow-up examinations in CSF and serum were performed in a female
patient with acute meningoencephalitis before and during HIV
seroconversion. RESULTS: HIV-ELISA, Western blot, and HIV-antigen in CSF
and HIV-ELISA in serum were negative on day 1. However, neopterin was
already significantly elevated in CSF (24.1 mumol/l) and serum (5.4),
compared with normal individuals (3.5 +/- 1.7). When HIV seroconversion
was documented by ELISA and Western blot on day 11, neopterin
concentrations reached even higher levels (133.6 in CSF and 21.5 in
serum). DISCUSSION AND CONCLUSIONS: Neopterin in CSF and serum may be an
important diagnostic marker in early HIV neuromanifestations, when HIV
testing is still negative in some cases.
